- Inflammation: CRP, ESR (ideally during flare and in remission)
- Hematology/chemistry: CBC with differential, CMP
- Urinalysis: protein/hematuria (screen for renal involvement and amyloidosis risk where relevant)
- Ferritin: particularly when systemic hyperinflammation is suspected
- Targeted tests: guided by phenotype (e.g., stool calprotectin, complement, immunoglobulins)
4) Decide when genetics is high-yield Genetic testing is most informative when paired with a well-described phenotype. Consider testing when onset is early, episodes are stereotyped, there is a strong family history, or the clinical picture suggests a monogenic syndrome. When ordering panels, document the phenotype and key negatives; it improves variant interpretation and downstream care. 5) Identify referral and escalation signals Escalate promptly when there is concern for organ-threatening inflammation or systemic hyperinflammation. Early collaboration with rheumatology/immunology can shorten time to diagnosis and reduce cumulative steroid exposure.Tip: Ask patients to keep a simple flare diary (date, temperature, symptoms, medications, response). It improves diagnostic clarity and supports shared decision-making.
